INSURANCE PLANS WE PARTICIPATE WITH

Eagle participates in most major insurance and managed care plans. However, health insurance and managed care plans do change, so please check the list below to see if Eagle contracts with your insurance carrier. If Eagle is not an in-network participating provider with your insurance plan, you can still be seen by an Eagle physician.
Effective 5-1-2019, Eagle will no longer file claims to non-contracted insurances, but will provide you a receipt and a summary of your services which will include all the information necessary for you to file to your insurance company for reimbursement. It is important that you know what your particular plan requires and what your financial responsibilities are. • Medicare Information You Need

Eagle is contracted as in-network, participating providers with the following insurance plans:

• Aetna* (all commercial products including Coventry, First Health,  Mail Handlers and all Medicare Advantage products)

• Aetna Medicare* (all products including Coventry Advantra) 

  * We are not in network for the Cornerstone products which require a Cornerstone PCP.

  * We are not in network with Aetna Medicare Prime HMO - POS which require a Wake Forest Baptist Health or Catawba Valley provider

• BCBS  We accept all Commercial products including Federal and NC State Health Plan. We are part of the Clear Pricing Project

  * We are not in network with Blue Local or Blue Home plans through Carolinas Heathcare, UNC Health Alliance, Novant, or Wake Forest Baptist.

        * We are not in network with BCBS High Performance Network

• Blue Medicare, including Healthy Blue Medicare

• Centivo

• Cigna - we accept all commercial plans including Great West, Cigna Oscar, Healthpartners Cigna Open Access, Cigna Connect

• Cigna Medicare 

• Coresource/Trustmark (will file all products)

• Devoted Health - Medicare Advantage Plan (effective 1/1/2023)

• Tom James/Fox Everett (for employees of Tom James Co.)

• Health Team Advantage

• Humana Medicare PPO, HMO, SNP. 

  *We are not contracted with commercial Humana plans

• Medcost Please look closely at Wake Forest Baptist / Cornerstone plans as not all have benefits outside of these providers.

• Medicaid (most providers are not taking new patients).  Eagle is contracted with Healthy Blue and UHC Community Plan Managed Care Plans. We are not in network with ANY out of state Medicaid plans.

• Medicare and Medicare Railroad

• OneNet PPO (formerly Alliance PPO)

• Primary Physicians Care

• Partners Direct Health Provider Network: Associated insurance plans are EBMS, Healthscope UMR, GPA 1360, American Plan Administrators

• Tricare East: Eagle is authorized to file, but we are non-par and not contracted.
