Eagle participates in most major insurance and managed care plans. However, health insurance and managed care plans do change, so please check the list below to see if Eagle contracts with your insurance carrier. If Eagle is not an in-network participating provider with your insurance plan, you can still be seen by an Eagle physician.
Effective 5-1-2019, Eagle will no longer file claims to non-contracted insurances, but will provide you a receipt and a summary of your services which will include all the information necessary for you to file to your insurance company for reimbursement. It is important that you know what your particular plan requires and what your financial responsibilities are. If you are uninsured and need assistance in finding insurance coverage, please call 800-318-2596 or visit www.healthcare.gov
